This prospective study evaluates the outcomes of tympanic membrane perforation repair in patients with inactive mucosal chronic otitis media (COM), comparing tympanoplasty alone with tympanoplasty combined with cortical mastoidectomy. Conducted from December 2022 to December 2023 at the Government Medical College, Latur, Maharashtra, India, the study involved 40 patients divided into two equal groups: Group A (Tympanoplasty only) and Group B (tympanoplasty with cortical mastoidectomy). Postoperative follow-ups were conducted at 2 weeks, 1 month, and 3 months to assess ear discharge, graft uptake, residual perforation, and hearing improvement. Group B demonstrated superior graft uptake (100%) compared to Group A (80%), and better postoperative air-bone gap (ABG) closure (19.54 dB vs. 12.46 dB, p< 0.05).
